Advanced Maternal Age Pregnancy (AMA): This refers to any woman that is either 35 years old or will be 35 years old at the expected time of delivery of her baby. The reality is that from a medical standpoint, a 35 year old woman is young. However, from a pregnancy standpoint they are older than the average patient. This age group is commonly considered “high risk pregnancy” for several reasons.

There are several conditions that are more common in women >35 such as pre-existing hypertension, diabetes, and heart disease. Women in this age group are also more likely to develop related pregnancy conditions such as preeclampsia, gestational diabetes, and peri-partum cardiomyopathy. These conditions will be discussed separately since they are not exclusive to advanced maternal age.

Although the mentioned conditions can be challenging during a pregnancy, the biggest concern for these patients is usually the risk of abnormal chromosomes (aneuploidy). In simplest terms, with an increase in age, there is an increase in the likelihood of having a wrong number of chromosomes in the egg that will eventually become the baby. This happens because women do not make new eggs during their life like they do with other cells of the body. In fact, all the eggs that a woman will have available during her lifetime are present by the time she is born. This prolonged arrested state of the eggs along with other time related factors, increase the chances of chromosomes separating unequally with advanced age.

How we manage an advanced maternal age patient depends on what the patient desires. At a minimum, these patients should be offered specialized screening tests during pregnancy to determine if the risk is high or low. Common tests available involve drawing blood from the patient and checking for certain hormone levels according to how far along they are in their pregnancy. More recently, advanced early ultrasounds can be performed to look at “nuchal translucency“, or the thin fat pad along the back of the neck of the fetus. This is particularly sensitive for Trisomy 21 (Downs Syndrome) babies that tend to have a thickened nuchal fold.

More recently, there has been a development of tests that detect tiny amounts of DNA secreted by the baby into the mother’s bloodstream. These tests claim a high detection rate for abnormalities, but are most sensitive when used together with the tests mentioned previously.

Detailed ultrasound (Level II) is another tool available to screen for chromosome abnormalities. A reasonable percentage of babies with Downs Syndrome, Edwards Syndrome, Patau Syndrome, and Neural Tube defects will have characteristic ultrasound findings.

Regardless of which test or combination of tests is used, patients need to understand that these are all screening tests, and as such, should not be used exclusively to make a diagnosis. Although these tests are very good at telling us when a patient does NOT have an abnormality, they are not as good at telling us when their is a problem. This is because these tests have a high false positive rate. It is important that patients understand this before undergoing testing, so that a positive test does not cause unnecessary anxiety.

So what happens with a positive test? The patient should be counseled and offered more invasive tests if they want to confirm a diagnosis. These tests are Chorionic Villus Sampling (CVS) and Amniocentesis. The choice of which is appropriate depends on how far along is the pregnancy. CVS is performed very early, but does have a higher rate of complications than amniocentesis which is performed a little later in pregnancy.

These tests involve the introduction of a needle into the uterus to obtain some of the fluid around the baby. This fluid is then analyzed for the correct number of chromosomes in order to either discard or confirm the findings of the preceding screening test.

Instead of “high risk”, I believe patients need to understand the proper term for their diagnosis and what are the medical implications of the diagnosis. There is no denying that patients are more and more educated on medical conditions and they deserve clear guidance on their medical condition.

As physicians, we have an obligation to communicate with patients and be prepared to answer questions in a way that patients may understand. When a patient understands the actual medical condition, they are more likely to make better treatment decisions and more likely to be compliant with their care plan. This has been our philosophy at Bayshore Womens Healthcare, and our patients agree.

Sea botox, busto o bum bum brasileño, Miami le encanta lucir bien. De hecho, los habitantes de esta gran ciudad gastan millones de dólares cada año en la búsqueda de la apariencia perfecta. Podemos discutir la ética, la presión social negativa y beneficios psicológicos de la cirugía estética sin fin, pero el hecho sigue siendo que la cirugía plástica es una parte bien establecida de la medicina moderna.

Sin embargo, existe otro ámbito de la cirugía estética que rara vez se encuentra en el centro de atención, pero está ganando popularidad ya que hoy los pacientes son más informados y abiertos a la discusión de cuestiones íntimas.

El término ginecología estética se aplica a varios procedimientos relacionados con los labios, apertura y cavidad de la vagina. A veces se comercializa bajo el término “rejuvenecimiento vaginal” pero el concepto es el mismo. Es cosmética en el sentido de alterar la apariencia, pero a diferencia de muchos procedimientos cosméticos, la mayoría de los pacientes también se benefician de una mejoría funcional. Las condiciones que se corrigen varían desde insatisfacción con la apariencia vaginal, las relaciones sexuales insatisfactorias, pérdida de orina, heces o relaciones sexuales dolorosas. Estas quejas son comunes, especialmente en pacientes con historiales de partos vaginales.

Ginecología tradicional ha focalizado el entrenamiento en cirugías que corrigen sólo la pérdida de orina o heces, sin un adecuado énfasis en apariencia o satisfacción sexual. Esto resulta en pacientes que mejoran sólo algunos de sus síntomas al someterse a la cirugía.

La realidad es que los pacientes son complejos y sus quejas son muy individualizadas. Lo que puede parecer como un problema menor para un paciente puede ser una condición que impacto profundo para otra. Un ejemplo común son los labios vaginales agrandados o desiguales. Aunque una variante normal en muchas mujeres, para algunas puede producir una imagen negativa de sí misma y en algunos casos puede causar dolor con cierta ropa o relaciones sexuales. En muchos casos las mujeres son reacias a discutir estas inquietudes con su médico o su pareja. El resultado podría ser una vida de infelicidad debido a un problema fácilmente corregible.

Pérdida de orina y la insatisfacción sexual son otras preocupaciones que muchas mujeres son renuentes a discutir. Tiendo a ver pacientes vocalizar estos temas cuando ya son mayores de edad, pero casi todos admiten que el problema comenzó varios años antes. A veces, la falta de relaciones íntimas que sean mutuamente satisfactorias resulta en problemas matrimoniales, especialmente cuando hay mala comunicación en pareja. Corrección de estos problemas tan pronto posible también produce mejores resultados ya que el tejido es más saludable y se sana mejor en las mujeres jovenes.

En resumen, ginecología cosmética es un término algo confuso que involucra mucho más que la apariencia física como el nombre implica. Sea dirigida como la ginecología estética, rejuvenecimiento vaginal o reparación vaginal, el cirujano debe individualizar el procedimiento para las necesidades del paciente. Avances en la medicina moderna son impulsados por el paciente; que se basa en el aumento del acceso a la educación médica. Nuestra sociedad se ha vuelto también más abierta a la discusión de lo que solía ser temas muy íntimos, a menudo tabúes. Ginecólogos progresivos tienen la responsabilidad de invitar a sus pacientes a una conversación abierta y a nunca descartar una queja del paciente como frívolo o sin importancia.

Whether its botox, breasts, or Brazilian butt lifts, Miami loves to look good. In fact, inhabitants of this great city spend millions of dollars every year in the search of the perfect look. We can debate the ethics, negative social pressure, and psychological benefits of cosmetic surgery all day, but the fact remains that plastic surgery is a well established part of modern medicine.

However, there is another realm of cosmetic surgery that rarely is in the spotlight but is gaining popularity as patients become more informed and open to discussion of intimate issues.

The term cosmetic gynecology applies to several procedures involving the lips, opening, and inside of the vagina. It is sometimes marketed under the term vaginal rejuvenation. It is cosmetic in the sense of altering appearance, but unlike many cosmetic procedures, the majority of patients also benefit from a functional improvement. The conditions that are addressed range from dissatisfaction with vaginal appearance, unsatisfactory sexual relationships, loss of urine or feces, or painful intercourse. Many of these complaints are the result of damage caused by child birth or due to pre-existing weak vaginal tissue.

Traditional gynecology has focused training on surgeries that address only loss of urine or feces, without an appropriate emphasis on appearance or sexual satisfaction. This results in patients having only some of their symptoms addressed when undergoing surgery.

The reality is that patients are complex and their issues are very individualized. What may seem as a minor problem to one patient may be a life altering condition to another. A common example is enlarged or uneven vaginal lips. Although a normal variant in many women, for some it can cause a negative self image and in some cases can cause pain with certain clothing or intercourse. Women are often times reluctant to discuss these concerns with their partner or her physician. The result could be a lifetime of unhappiness due to an easily correctable issue.

Loss of urine and sexual dissatisfaction are other concerns that many women are reluctant to discuss. I tend to see patients vocalize these issues when they are older, but almost all will admit that the issue began several years before. Sometimes, the lack of mutually satisfactory intercourse results in marital problems, especially when there is poor communication as a couple. Correcting these issues as early as possible also produces better results since the tissue is healthier and heals better.

In summary, cosmetic gynecology is a somewhat confusing term that involves much more than physical appearance as the name would imply. Whether addressed as cosmetic gynecology, vaginal rejuvenation, or vaginal repair, the surgeon needs to individualize the procedure to the patient’s needs. Advances in modern medicine are driven by the patient; who relies on the increase in access to medical education. Our society has also become more open to discussions of what used to be very intimate, often taboo topics. Progressive gynecologists have a responsibility to invite open conversation with their patients and to never dismiss a patient complaint as frivolous or unimportant.
